Three leaves with preprinted staff lines, handwritten in pencil on the front sides. The document is in good physical condition. The document was laminated. There is lamination-related creasing throughout the document. There is minor fraying along the top edge of the first leaf. A small portion of the upper left corner of each leaf was torn away. There is a blue ink stain at the upper left corner of each leaf. There is a paper clip rust stain at the upper left corner of the first leaf. There is a date stamped in black ink at the upper left corner of the first leaf. There is a boxed "A" stamped in blue ink at the upper left corner of the first leaf. There is a handwritten check mark at the lower right corner of each leaf.
