Two leaves with preprinted staff lines, handwritten in black ink on both sides. The document is in fair physical condition. The document was removed from a bound volume. There is moderate fraying at the right edge of the first leaf and at the left edge of the second leaf. The document was laminated. The upper left corner of the first leaf was folded into the laminate. There is a major tear across the middle of the first leaf. There is a moderate tear in the center of the first leaf. There is a significant tear at the middle left edge. This tear was repaired with transparent adhesive tape. There are other minor tears on the edge of the first leaf. There is a major tear across the middle of the second leaf. This tear was repaired with transparent adhesive tape. There are moderate tears at the left and right edges of the second leaf. There is a minor tear at the right top edge of the second leaf. There is a handwritten "x" at the top left corner of the second leaf. There are handwritten check marks at the lower right corner of each leaf. An unknown document is located on the reverse side of the first leaf. The document "My Pretty Quadron," Sheet Music 9 is located on the reverse side of the second leaf.
