One leaf, handwritten in black ink on both sides. The document is in good physical condition. The document was laminated. There is a moderate tear hole to the right of the document title. There are two small tears at the upper edge. There is a small tear at the left bottom edge. There are two small tear holes near the middle of the document. The document was folded horizontally and vertically into sixths, possibly for mailing.
