Three leaves of lined paper, handwritten in black ink on the front sides. The document is in good physical condition. The document was laminated. There are three holes across the top edge of each leaf. There is a moderate tear at the upper right corner of each leaf. There is minor fraying along the top edge of each leaf. There is minor staining of unknown origin throughout the document. The lower right corner of the second leaf was folded into the laminate.
