Two leaves of lined paper, handwritten in black ink on both sides. The document is in good physical condition. The document was laminated. There is minor fraying along the top edge of the first leaf. There is moderate fraying along the top edge of the second leaf. There is a small tear at the center bottom edge of the second leaf. The document was vertically folded in half.
