One leaf with preprinted staff lines handwritten in black ink on both sides. The document is in good physical condition. The document was laminated. A torn flap at the left upper edge and a torn flap at the middle left edge were folded into the laminate. There are numerous tears of varying severaity along the edges of the document. There is a substantial portion torn away from the middle lower edge of the leaf. There are numerous stains of unknown origin throughout the document. There is a handwritten check mark to the right of the musical notations. The documents "Bonny Barbara Allan," Sheet Music 8, "The Wife of Usher's Well," Sheet Music 12, "The Wife of Usher's Well," Sheet Music 13, "The Lexington Murder," Sheet Music 8, and "The Blind Girl," Sheet Music 1 are located at the bottom of the leaf. The documents "The Sweet Trinity (The Golden Vanity)," Sheet Music 8, "Lamkin," Sheet Music 7, "Sweet Forget-Me-Not," Sheet Music 2, and "I'll Hang My Harp on a Willow Tree," Sheet Music 3 are located on the reverse side of the leaf.
