One leaf with preprinted staff lines handwritten in black ink on the front side. The document is in good physical condition. There are pad perforations along the upper edge. The document was laminated. The upper left corner was folded into the laminate. There are paper clip rust stains at the upper left corner. There is a handwritten classification note at the upper right corner. There is a handwritten check mark at the lower right corner.
