Two leave with preprinted staff lines, handwritten in pencil and carbon on the front side. The leaves are glued together, one above the other. The document is in good physical condition. The document was laminated. There is lamination-related creasing on the document. The upper right corner and the lower left corner were folded into the laminate. There are paper clip rust stains at the upper left corner. There is a handwritten check mark at the lower right corner. The first (upper) leaf is a carbon duplicate.
