Two leaves, typewritten on the front sides. The document is in good physical condition. The document was laminated. The upper left corner of the first leaf, the upper left corner of the second leaf, and the lower left corner of the second leaf were folded into the laminate. A portion of the bottom edge was cut away from each leaf. There is a paper clip rust stain at the upper left edge. here are staple-related and paper clip related holes at the upper left corner of each leaf. There is a minor tear at the left upper edge of the second leaf. The document was folded horizontally in half. There is a note handwritten in pencil at the top right corner of the first leaf. Stanza numbers are handwritten in black ink on the first leaf.
