Two leaves, typewritten in blue ink on the reverse side of I. G. Greer's departmental letterhead. The document is in good physical condition. The document was laminated. The upper right corner was folded into the laminate. There is a moderate tear at the middle bottom edge. The document was folded horizontally and vertically into eights, possibly for mailing. There are two textual corrections handwritten in black ink.
