Description
One leaf of lined paper, handwritten in black ink on both sides. The document is in good physical condition. The document was laminated. There are two loose leaf binding holes along the left edge. The upper right corner was folded into the laminate. There are two minor tears at the top edge. The document was folded both horizontally and vertically into sixths, possibly for mailing.
