Four leaves of lined paper, handwritten in black ink on the front sides of leaves one through three and on the front and reverse sides of leaf four. The document is in good physical condition. The document was laminated. The first and second leaves were laminated together back-to-back. There is a significant piece torn away from the upper right edge of each leaf. There is a moderate piece torn away from the right top edge of each leaf. There is a paper clip rust stain at the middle top edge of the first leaf. There is a paper clip rust stain the middle top edge of the fourth leaf. The upper right corner and the lower left corner of the first leaf, the lower left corner of the third leaf, and the lower left corner of the fourth leaf are all folded into the laminate. The document was vertically folded in half. There is a note handwritten in pencil at the upper right corner of the first leaf. The informant citation is handwritten on the fourth leaf.
