Two leaves of lined paper, handwritten in pencil on both sides. The document is in good physical condition. The document was laminated. The document was folded horizontally and vertically into sixths, possibly for mailing. There are numerous folding-related minor tears throughout the document. The upper right corner of the second leaf was folded into the laminate. The informant citation is handwritten on the reverse side of the second leaf.
