One leaf typewritten on the front side. The document is in good physical condition. The document was laminated. The lower right corner was folded into the laminate. There is a minor tear at the right lower edge. There are paper clip rust stains at the middle upper edge. There are black ink stains throughout the document. There are two textual corrections handwritten in black ink. The informant citation is handwritten in black ink at the lower right corner.
