The document was removed from a bound volume. There is significant fraying along the right edge. The document was laminated. There is a major tear at the left upper edge. There are also two minor tears at the left upper edge. There is a minor tear at the right lower edge. There is handwritten check mark at the upper right corner. There is also a parenthetical "4" handwritten at the upper right corner. There is a handwritten check mark at the lower right corner. The document is located on the reverse side of "I Wish I was a Single Girl Again," Sheet Music 5.
