The upper right corner, the lower right corner, and the lower left corner were folded into the laminate. There is minor tearing at the lower right edge. The document was folded horizontally and vertically into quarters. There is an editorial note and/or informant citation handwritten in pencil at the upper right corner. There are numerous textual corrections handwritten in pencil.
