One leaf with preprinted staff lines handwritten in black ink on the front. The document is in good physical condition. The document was laminated. There are some lamination-related creases at the upper right corner. The upper left corner and the upper right corner were folded into the laminate. There are staple holes at the upper left corner. There are pad perforations along the top edge. There is a handwritten check mark at the lower right corner.
