One leaf with preprinted with staff lines handwritten in black ink on the front side. The document is in excellent physical condition. The document was laminated. There are pad perforations along the top edge. The upper left corner and the left lower corner are folded into the laminate. There is a handwritten check mark in the lower right corner. There is one editorial note handwritten in pencil.
