One leaf with preprinted staff lines handwritten in black ink and pencil on the front side. The document is in good physical condition. The document was laminated. There is a minor tear at the lower left edge. There is a minor tear at the center bottom edge. There is a significant tear at the right lower edge. The lower left corner was folded into the laminate. There are pad perforations along the top edge. There is a boxed "A" stamped in blue ink at the upper left corner. The date is stamped at the left top edge. There is a handwritten check mark at the lower right corner. The document was folded horizontally and vertically into fourths.
