One leaf with preprinted staff lines handwritten in carbon on the front side. The document is in excellent physical condition. The document was laminated. There are two sets of staple holes at the top edge. The holes have minor rust stains. There is a blue ink stain at the upper left corner. The upper right corner was folded into the laminate. There are two minor tears at the lower left edge. There is a check mark at the lower right corner following the musical notes. The document "Dives and Lazarus," Sheet Music 04, Copy 1, Page 2 is located at the bottom of the leaf.
