There is minor fraying along the left edge. A portion of the lower left corner was torn away. The document was laminated. There is a minor stain of unknown origin. There is a parenthetical "5" handwritten in pencil at the upper right corner. There is a check mark handwritten in black ink at the lower right corner. The document "America the Beautiful," Sheet Music is located on the reverse side.
