The upper left corner, the lower right corner, and the lower left corner were folded into the laminate. There are two sets of staple holes at the upper left corner. There is one minor tear at the left upper edge. There are four minor tears at the left edge. There are stains of unknown origin on the document. There are several editorial marks handwritten in pencil. The document appears to be a carbon duplicate.
