One leaf of lined paper, handwritten in black ink on both sides. The document is in good physical condition. There are two loose leaf binding holes along the right edge. The document was laminated. There is a moderate tear at the lower right corner. The document was folded vertically in half. The [possible] informant citation is located at the upper right corner. The text of an academic assignment is located on the reverse side.
