Description

Two leaves of lined paper, handwritten in black ink on the front sides and in pencil on the reverse side of the second leaf. The document is in good physical condition. There are two loose leaf binding holes along the left edge. The document was laminated. The upper left corner of the second leaf was folded into the laminate. There are lamination-related creases throughout the document. There are staple holes and associated rust stains at the middle upper edge of both leaves. There are minor stains of unknown origin throughout the document. The document was folded vertically in half, possibly for mailing. The word "copy" is handwritten in pencil at the upper right corner of the first leaf.
