One leaf, typewritten on both sides. The document is in good physical condition. The document was laminated. There are several lamination-related creases on the document. The upper right corner was folded into the laminate. There is a moderate tear at the upper right corner. There are three minor tears at the left lower edge. There is a moderate tear at the middle left edge.
