Description
Two leaves of lined paper, handwritten in black ink on the front sides. The document is in good physical condition. The document was laminated, with the leaves back-to-back. The upper left corner of the first leaf was folded into the laminate. There is minor fraying along the top edge of the first leaf. There is acid-related staining at the top edge of both leaves. The document was folded vertically into thirds, possibly for mailing. There is a note handwritten in pencil at the upper right corner of the first leaf.
