Two leaves, handwritten in black ink on both sides of the first leaf and on the front side of the second leaf. The text on the reverse side of the second leaf is handwritten in pencil. The second leaf is lined paper. The document is in good physical condition. The document was laminated. There are numerous lamination-related creases throughout the document. There is a moderate tear at the left top edge of the second leaf. There is minor fraying at the upper left corner of the second leaf. The document was folded horizontally and vertically into quarters, possibly for mailing. There are numerous textual corrections handwritten in pencil.
