Description
Six leaves, handwritten on the front sides. The document is in good physical condition. The document was laminated. There are staple holes at the center top edge of each leaf. There is a moderate tear at the upper left corner of each leaf. There is a moderate tear at the middle bottom edge of the fifth and sixth leaves. There is a minor tear at the middle top edge of the sixth leaf. Several corners throughout the document are folded into the laminate. There is a black ink stain at the middle bottom edge of the first leaf. There are minor stains of unknown origin throughout the document. The informant citation is located on the sixth leaf.
