Two leaves, typewritten on the front sides. The document is in good physical condition. The document was laminated. The upper right corner and both lower corners of the first leaf were folded into the laminate. There are staple holes at the upper left corner of each leaf. There are paper clip-related rust stains at the upper left corner of the first leaf. There is a moderate tear at the upper right corner of each leaf. The document was horizontally folded into fourths, possibly for mailing. There are three editorial marks handwritten in pencil. There is a pencil mark of unknown significance in the upper left corner of the first leaf.
