Three leaves of lined paper, handwritten in pencil on the front sides. The document is in good physical condition. The document was laminated. There are several minor edge tears throughout the document. There is a moderate tear at the right top edge of the first leaf. The top right corners of the second and third leaves were folded into the laminate. There are numerous stains of unknown origin throughout the document. The document was horizontally folded into thirds, possibly for mailing. One informant citation appears above the title on the first leaf. A second informant citation follows the song text on the third leaf.
