Description

One leaf of lined paper, handwritten in blue ink on both sides. The document is in excellent physical condition. The document was laminated. There is a moderate ink stain at the upper right edge of the reverse side. The document was folded horizontally and vertically into sixths, possibly for mailing. The informant citation is handwritten in pencil at the upper right corner of the front side.
