Description
Two leaves, typewritten in blue ink on the front sides. The document is typed on the reverse of Appalachian. Training School Department of History stationery. The document is in good physical condition. The document was laminated. There is a moderate tear at the upper left edge of the first leaf. There are several minor tears along the edges of the second leaf. There are paper clip-related rust stains at the center top edge of each leaf. The document was folded vertically in half. There are some minor stains of unknown origin on the document.
