Two leaves, typewritten on the front of each leaf. Both leaves are in good overall condition. Both leaves exhibit minor tears along their right edges. Transparent adhesive tape was applied to the upper left corner of both leaves, apparently in an attempt to repair damage resulting from the removal of a staple. A brief note from the collector is handwritten on the first leaf.
